How to Submit for Gold Bundle:

1.    Fill out the Educators Student Group Exam Bundle Form in its entirety. Make sure not to hit submit until you have put in all the required information.

2.    Do not send payment; wait until AST sends you an invoice verifying correct payment total. Incorrect payment will be returned which will result in unnecessary delays for your students to receive their exam materials.
AST will notify you of the payment total by email within 7 business days from the date the forms were received.

3.     The exam bundles are processed within 5 business days upon receipt of payment.

Student Information Form:

This form is for gathering the required information from each student that you will need to complete the Educators Student Group Exam Bundle. This is only for the program’s use and not for AST.

Forms of Payment That are Accepted or Not Accepted:

1.  Credit Cards: Visa, Master Card or American Express
(Due to PCI compliance, AST CANNOT accept credit card payment information by email, you can mail or call in your credit card information.)

2.  Checks: Cashier’s check, money order or institutional check, payable to AST.

3.  NOT accepted: Purchase orders, student personal checks, or Discover cards.

Important Information:

1.  Students who already have an AST membership will get another year of membership added. Membership is non-refundable. Please inquire if your student(s) purchased a student membership prior to filling out this form and what their membership ID# is.

2.  Students who already purchased the gold bundle are NOT eligible to purchase another gold bundle.  

3.  If student is paying by a personal credit card and it declines, and a new form of payment has been processed, the students’ study guide will be sent directly to the student.

4. AST processes and ships the study guides to programs by FedEx Ground. Delivery occurs within five to seven days. If rush delivery is required, additional charges apply.

5. The National Board of Surgical Technology and Surgical Assisting (NBSTSA) does impose a deadline on the prepaid certification exam. Please contact the NBSTSA for more information. They can be contacted at 1-800-707-0057 chose option 3 or by email at [email protected]
